It looks like he has serrated the ridge around his glans. I've seen single gaps like that after a dydoe migrated out, but this mod is new to me.

All healed now, took about 4 weeks, i love how it feels, think i will extend the cuts a bit further at some point so i have longer flaps and i intend to add piercings to the flaps
