I only have a partial meato, but I have deliberately done it down the front of the cock head rather than down the back. I did it that way because I too have a PA and again wanted to keep it intact. I have only got my PA up to 10mm, but have to say that there is no way I would have got a ring that size in through my pee hole if I hadn't cut the head down the front. I also doubt that I would have been able to pee if the ring was in that tightly!
