How did it 'reject'? It didn't tear out, did it?

The skin got thinner and thinner until one day having a bit of fun with the misses it did just that... not much blood. But since then I've done a lot of saline and pumping and haven't quite got the courage up to replace it. I'd have to "grab" quite a lot of tissue and go down the urethra quite a bit I think to make sure it never happens again.

You could do something like this. Although it'd be pretty difficult getting it all inside the pussy.

I've read that there are urologists who can repair a tear-out, but I have no idea what such a procedure would cost. Going deep looks like it would be a lot cheaper.

Something you may have already considered - a carabiner instead of a ring or bent barbell. Apparently it's one of those passing fads - there are a bunch of them on Reddit - but the straight section on the carabiner would put less tension on opening and the fistula, and you can get them in stainless steel.

Got rid of my frenulum many years ago and a partial meato, 5mm, just to allow my fave sounds to go in easier for stimming.

Frenulum removed, meatotomy well advanced to underside of shaft to facilitate sounding and stretching needs and now gradually cutting the glans upwards. Addictive
