Werebear here, very long time lurker (20 years???) first time post and a question to Tetherspout himself. Read about your products after seeing a guy in a chastity cage using a spout to secure himself in it and then finally found your website with your products and thoughts. By the way I am deeply impressed with your design.
My question is would a Tetherspout work on a guy with a PA piercing who has sounded up to 13mm? I have stretched my PA to 8mm in the past but it is now about 6mm again. I have tried to use my PA to put some tension on my cock but not really a good idea as it can very easily tear and I have first hand experience of this. My partner lost his when it got caught in my trouser belt loops when we were playing horsey once. Possibly TMI but if it is then I'm probably in the wrong place!
Anyway, I rope my cock with a noose and that is fun play but sometimes isn't the sensation I want and long term play isn't possible without tweaking the noose every now and then.
So what I am asking is this simply a sizing issue or will I have changed the shape of my urethra? Have any guys with PA's tried these fantastic devices?

Theoretically, a PA piercing shouldn't have any effect on being able to wear a tetherspout, provided the piercing is situated below where the retainer will sit and hasn't torn or migrated out. As long as the retainer doesn't conflict with the location of the piercing, which will usually be the case, there shouldn't be any problem.
In your case, however, there could be a complication. The fact that you can sound up to 13mm means that you might need a retainer as large as 39Fr, in which case you'd have to make it yourself because I don't have anything that large. But more concerning is whether your fossa navicularis will be large enough to comfortably hold a retainer that large.
Let me explain. Tetherspouts rely on the retainer being able to loosely and comfortably reside within the fossa navicularis without causing any outward pressure or distension. However, there is no easy or practical way to measure the size of the fossa navicularis. So fitting of retainers is instead based on the size of the urethral opening. This is done based on the empirical observation that if it will fit through the urethral opening, most times the retainer will have no trouble fitting in the fossa navicularis after it is turned 90 degrees. Even the largest oval retainers will usually fit. But if the orifice was previously stretched, which I strongly suspect happened in your case because 13mm is unusually large, this size relationship may no longer hold and fitting may require enlarging the fossa navicularis or using a smaller retainer. Enlarging the fossa navicularis is not difficult, but requires determination and may involve some discomfort. The only way to know for sure is by trying.

Just as a data point: my natural, unstretched urethra easily accommodated a 13mm diameter sound. So while out of the ordinary and for most guys it's as a result of stretching, it's not technically impossible...

Thanks for getting back Tetherspout. I had a feeling my fun with sounds might exclude me from a spout and to be honest I'm not done with sounds - one of my tops is fond of pushing his fingers into my urethra which is wild. Hi soundsgreat87, thanks for responding. I think Tetherspout is saying that because I stretched to 13 mm that I've probably stretched the opening so it wont hold a retainer. The fact that you can accommodate a 13 mm sound naturally is amazing by the way!
