The human body is surprisingly elastic, especially over long periods of time.
Usually, you see this type of stuff in morbidly obese folks, but it gets completely hidden under the rolls of fat.
Its possible to happen, though I wouldnt know the circumstances to cause it to locally buildup there specifically. It appears to be filling with fluid at a rate slow enough to stretch and expand with new skin. But not fast enough to migrate out of the scrotum into the abdominal cavity.

The reason I posted this in the impotence forum was because it is indeed a natural phenomena and not the result of drugs or injections or saline or silicone inflation or pumping. Also this man doesn't have access to his penis so I figured it sort of fit here. It's absolutley real. Here is the link to the scientific journal article where it's from. There are many such cases.
