You try to do an Ampallang - if i understood you right.
I used emla Cream too- (acc. to the advice - Leaflets - in the Box)
I used a clamp to fix the Head of my cock and I pushed the needle through.
You may need to push some Cream inside of your urethra than it could work from the inside too.
You can use a syringe to fill the urethra.
The adrenaline makes it much more comfortabel than expected and you could feel if theres something wrong.
Make a short break and go on whit the needle.
In Germany you could by the Emla cream in every drugstore.

Ok, I did my ampallang myself. I devised a system using a small drill press. Since my ampallang is a 4 ga. the drill press allowed for the pressure necessary to push through a custom built leather punch. The sensation, of pushing the leather punch through was almost orgasmic. Breaking the surface of the penis head, both the entering and exiting was the only painful experience. Bleeding was controlled by tightly binding the penis head. This does not stop the bleeding but minimizes the bleeding. The punch was cleaned and sterilized in a hot oven. I had no infection from this procedure. Proper care after the piercing is vital to the success of the piercing.
