Is this caused from using to much of a vacuum?

Yes !

Yeah you can get them for a combination of reasons. But the most common is too high of pressure or in the tube too long without a break is another one. And as mentioned above. When you pack a tube most of the vacuum pressure ends up on the head of you penis and it makes it all that much easier to overdo it. Generally once I fully pack a tube, I back the pressure way back as a precaution.
