I put the bra on after. Wearing a bra during the inflation would compromise sterility to me. You don't want all that fabric and rubbing near the infusion site.

I put the needles (catheters) in just under the skin. I use 1 1/4" inch 22ga catheters. The bigger ones hurt more to put in, and I can't infuse any faster since the infusion rate is usually restricted by pain. Faster=more painful. I put them in pointing down about an inch above and inside the nipple, then put a big 2x4" sterile bandaid over it to hold it in place. Then use either medical tape or bandaids to hold the tubes down on the chest.

Keep the infusion and the play (bra, etc) separate. The more medical and sterile you can make the infusion, the safer it is.
